I suggest hitting up the people who do this. You can find them on this forum as well I cant image this stuff being expensive. Maybe they can sell you a template or fill in for you.
Time is money and you can''t do everything yourself, leave it to professionals and to save couple dollars you end up waisting couple days, looking for it - it's just not worth it in my opinion.
