Take Plastic card
Print out the photoshop id docs.
Add it on Plastic card take Plastic membrane add on top the Plastic card one the printed paper
Make Photo , and done

If you want hologram then add Plastic mbrane under hologram... And add one Plastic membrane on top of the hologrom. Use thicker Plastic membrane for this
